CHOICES – INTENSIVE PSYCHIATRIC REHABILITATION TREATMENT – (IPRT)

IPRT is a highly focused time-limited outpatient program which is designed to enable adults with a mental illness to form and achieve goals in regards to their living, learning, socializing, and working environments.  516-867-5813

The program provides the following services:

- Psychiatric rehabilitative readiness determination

- Psychiatric rehabilitative goal setting

- Psychiatric rehabilitative functional and resource assessment

- Psychiatric rehabilitative service planning

- Psychiatric rehabilitative skills and resource development

- Discharge planning and community referrals
