Delmont Medical Care
This initiative addresses the overuse of emergency services caused by patients who typically seek their health care services in a hospital setting rather than through a primary physician. It was developed as part of the New York State Department of Health’s Delivery System Reform Incentive Payment (DSRIP) Program to improve access to primary care services by locating clinics adjacent to community EDs.
Dr. Delmont is the founder of Delmont Medical Care, a multisite, multispecialty, NCQA level 3 medical home practice that delivers culturally sensitive, efficient and high-quality care to thousands of low-income, uninsured and underinsured patients. Mercy ED patients without a primary care physician are now referred to Delmont Medical Care, which also provides internal medicine for the hospital’s Family Care Center.
