Primary care is where your path to wellness starts. Your PCP is your first contact for general health questions, annual check-ups, vaccinations and illness. It’s important to have a PCP who knows your medical history and can coordinate care with specialists when needed.

Your PCP is your medical home—a way of providing primary care that is patient centered and team based with a focus on coordination as well as quality and outcomes. If you require more specialized care, your Catholic Health primary care provider will refer you to our vast network of experts or other resources across our continuum of care, such as home health, rehabilitation services, or hospice care.

But your PCP will always be there—as your medical home—to facilitate your health care needs at Catholic Health.

Primary Care FAQs

A primary care physician (PCP) is your main general doctor. They can be in family medicine, geriatrics, internal medicine or pediatrics. They are your primary contact for annual check-ups, preventive care, illness or general health questions.

Preventive care helps you stay healthy by preventing and catching conditions like diabetes and heart disease early. We perform preventive screenings for certain conditions if we suspect you are at risk of developing them. Early diagnosis helps us take the best possible care of you.

Catholic Health’s primary care physicians will be your point of contact for your everyday health needs. We will make sure you understand all the elements of your care plan, including why we are recommending certain medications, treatments or health screenings. At Catholic Health, we provide information to empower you to make decisions about your health.

Our primary care physicians seamlessly and quickly coordinate your care. They can provide referrals to specialists, radiology centers, physical therapists and other health services within Catholic Health whenever you need them.

Your primary care provider will arrange for urgent care or make a referral to Catholic Health’s Patient Navigation Hub for all other care.

The Patient Navigation Hub is staffed with a team of specialized patient navigation coordinators to manage your referral, ensure you are scheduled with the right provider, and schedule a timely appointment. You can also use MyChart to make appointments.

The Patient Navigation Hub provides you with:  

•    A dedicated patient navigation coordinator
•    Assistance in finding nearby doctors and specialists who can best care for your needs
•    One-stop coordination of all your appointments and medical needs—in-person or telehealth visit with Catholic Health Video Visits
•    Scheduled appointment reminders and reminders for annual screenings
•    Catholic Health MyChart online access to your medical records and test results
•    Two-business day turnaround for routine referrals and same-day turnaround for urgent referrals

On your first visit to a Catholic Health primary care provider, you will talk about your personal and family health history. A nurse will take your temperature, blood pressure and pulse as well as measure your height and weight. Your physician will perform a physical exam and talk with you about your health. This visit is a great time to discuss your personal health goals.

Questions may include:

  • How do you feel about your overall health?
  • What is your level of physical activity?
  • In order, what are your top three major health concerns?
  • What is your personal health goal and what have you done so far to achieve it?

Catholic Health MyChart is a secure portal to keep our patients educated and engaged in their own health care. Through Catholic Health eHealth, our secure electronic medical record (EMR), physicians and other health care providers work from a single, shared health record for every patient, using the Epic EMR system.

Our primary care practice office hours vary by location. Most are open Monday to Friday from 8:00 a.m. to 8:00 p.m. Some are open on Saturdays. Virtual care is offered after hours and on weekends as well as normal business hours.
 

Care such as IV administration is not currently performed at our primary care practices. Your PCP will refer you to Catholic Health’s Home Care unit to receive IV administration or like services in your home. 

Catholic Health and its Accountable Care Organizations (ACOs), under the management of Catholic Health Physician Partners, improves the care for 38,300 Medicare beneficiaries. Our quality performance exceeds 98 percent, which means better care for Long Islanders.

Our physicians screen patients for social determinants of care. If an issue is identified, patients are referred to Catholic Health Physician Partners’ care coordination team.
